SimplePractice pricing in 2026: every plan, every add-on, and the bill you actually get
Starter $49, Essential $79, Plus $99, plus $74 per extra clinician, $35 Note Taker and 3.15% card fees. The year one and year two math, side by side.
October 28, 2026 · 9 min read
SimplePractice lists three plans on its US pricing page: Starter at $49, Essential at $79 and Plus at $99 a month, with each additional clinician at $74 a month on the Plus plan only. The AI Note Taker is $35 per clinician per month. Care Aide, the AI assistant bundle that includes Note Taker, is listed at $59 for the account owner and $49 for each additional clinician. ePrescribe is $49 a month plus an $89 one time setup fee. Card processing is 3.15% plus $0.30. Groups of six or more clinicians are sent to sales for custom pricing. Those figures were read on SimplePractice's pricing page on October 28, 2026.
There is one more number on that page that changes everything about your first invoice: 50% off the first three months if you start a paid account within seven days of your trial, plus 30 days free on Note Taker and 45 days free on Care Aide. So a two clinician practice that thinks it signed up for $173 a month pays $86.50 for three months, then $173 for the rest of its life. The price you remember is not the price you pay, and that is worth doing the arithmetic on before you migrate your charts.
The published price list
| Item | Monthly list price | Notes |
|---|---|---|
| Starter | $49 | one clinician, no additional clinicians available |
| Essential | $79 | one clinician |
| Plus | $99 | first clinician; the only plan that takes a team |
| Additional clinician (Plus) | $74 each | per clinician, per month |
| AI Note Taker | $35 per clinician | 30 days free, then a promotional $17.50 |
| Care Aide | $59 owner, $49 each additional clinician | includes Note Taker, 45 days free |
| ePrescribe | $49 | plus an $89 one time setup fee |
| Card processing | 3.15% + $0.30 | per transaction |
| Launch promotion | 50% off for 3 months | paid account started within 7 days |
Read the second row twice. Additional clinicians exist only on Plus. If you are two clinicians and you were pricing Essential at $79 because that is the plan with the features you wanted, your actual entry point is $99 plus $74, which is $173, before the scribe. That jump from $79 to $173 is where most of the surprise in this market lives.
Year one against year two
The promotion is real money and it is also a reason people underestimate their own software budget by a third. Here is a three clinician group on Plus with Note Taker for everyone, using list prices and the published 50% off three months on the base subscription.
| Line | Monthly | Year one | Year two |
|---|---|---|---|
| Plus base, 3 clinicians ($99 + $74 x 2) | $247 | $2,593.50 | $2,964 |
| Note Taker, 3 clinicians at $35 | $105 | $1,260 | $1,260 |
| Subscription total | $352 | $3,853.50 | $4,224 |
Year one comes in about $370 cheaper than year two on the base alone, and that is before counting the free Note Taker month. Nothing dishonest is happening here, the terms are printed. But if you build your budget from the first invoice you will be short, and if you are comparing vendors you need to compare steady state prices or the comparison is meaningless.
Now add cards. At $150 a visit and 240 visits a month across three clinicians, 3.15% plus $0.30 comes to $1,206 a month, $14,472 a year. The subscription is 21% of what you pay SimplePractice. People spend three weeks choosing between a $79 plan and a $99 plan and never once open the processing statement.
What each plan actually includes
The broad shape, from the pricing page and its feature grid:
- Starter, $49. One clinician. Calendar, notes, telehealth, billing basics, client portal. No additional clinicians, ever. It is a solo plan with a ceiling, and the ceiling is the plan itself, not a usage cap.
- Essential, $79. Still one clinician, with the fuller feature set most practices want day to day.
- Plus, $99. The group plan. Multiple clinicians at $74 each, team permissions, the preferred phone support line.
Support is included on all three plans, with the priority phone line attached to Plus. That is normal. Every vendor in this bracket includes support now, including us, so "support included" belongs in nobody's pitch deck.
Where SimplePractice is the right answer
I compete with them and I will still tell you when to buy them. Three situations:
You bill insurance. SimplePractice has claim submission, a large payer network, eligibility checks and ERA handling. Our software has none of it. No clearinghouse, no eligibility verification, no denial workflow. If payers are a meaningful part of your revenue, this is not a close call and no price table changes it.
You want the ecosystem. It is one of the largest behavioral health platforms in the United States. That means a deep library of note templates, an enormous amount of community knowledge, integrations that already exist, and billers who already know the interface. When you hire, the odds are decent your new clinician has used it before. That is a real operational saving that does not show up on a price sheet.
You need e-prescribing in one system. ePrescribe at $49 plus the $89 setup is in the product. We do not do e-prescribing at all. In states with an e-prescribing mandate, including California, Florida, New York, Michigan, Minnesota and Delaware, our users run a separate e-Rx platform, and we say so before anyone pays us.
Where it gets expensive, and what people switch to
The per clinician charge is the pressure point. At one clinician, Starter plus Note Taker is $84 and that is competitive with anything. At three clinicians the same stack is $352. Nothing about the software got four times better; you added two seats.
For context, here is the same comparison at one, two and three clinicians, using list prices read on public pricing pages on October 28, 2026. Check them yourself, they move.
| Setup | One clinician | Two clinicians | Three clinicians |
|---|---|---|---|
| SimplePractice with Note Taker | $84 (Starter) | $243 (Plus) | $352 (Plus) |
| TherapyNotes with TherapyFuel | $109 | $209 | $299 |
| Practice Better Team with AI | not needed, $79 on Grow | $225 | $285 |
| IntakeQ / PracticeQ, no AI scribe available | $84.90 | $114.90 | $144.90 |
| Our Clinic plan with AI scribe | $69 (Essential plan) | $159 | $179 |
Our figures are the annual rate billed monthly; month to month is $59 for Essential and $143 for Clinic. And to be fair about it, at a single clinician we are not the cheapest option on the market: Noterro is $33, Carepatron $39 and Practice Better $35 on Launch. The gap opens up at two and three seats, because our Clinic plan covers one to three users for $119 and only the AI scribe is charged per head at $20. The full list is on our pricing page and the head to head is on our comparison page.

People searching for SimplePractice competitors usually fall into two camps. If you bill insurance, your real shortlist is TherapyNotes, Jane and the insurance focused tools, and you should be comparing payer network and claim scrubbing, not dollars. If you are self-pay, you are paying for a claims engine you never open, and the honest list of alternatives is shorter and cheaper. We wrote about that trade in choosing an EHR for a small or solo practice.
The things people get wrong on the invoice
Four patterns I see constantly when practices move their billing over to us and finally read their old statements:
- The promo ends in month four and nobody updated the forecast. That $370 on a three clinician group is a line item, not a rounding error.
- Note Taker is per clinician, not per account. Six clinicians at $35 is $210 a month, $2,520 a year, for note drafting.
- Card fees are never in the comparison spreadsheet. 3.15% plus $0.30 against 2.6% plus $0.10 at the front desk is hundreds of dollars a year on identical revenue.
- The clinician count used in the trial is not the clinician count in month six. Price the practice you expect to have, not the one you have today.
What we do not do, so you can rule us out fast
Our software does not bill insurance, and that is a design decision rather than a roadmap item. No clearinghouse, no eligibility, no denials. It is not ONC certified either, which only matters if you report to Medicare or MIPS; we went through why that is true in our piece on ONC certification for cash-pay practices. No e-prescribing. No built in CPT database, because the AMA licenses that code set, so your clinic enters its own codes. What it does do for a self-pay practice is produce a complete superbill PDF with NPI, EIN, CPT and ICD-10 codes and the patient balance, straight from the visit you just charted, at no extra cost.
If you need claims, buy SimplePractice. If you hand the patient a receipt and they file it themselves, you are paying for a claims engine you will never log into, and the arithmetic above is the argument. The longer version, including where they beat us, is on our SimplePractice alternative page.

Frequently asked questions
How much does SimplePractice cost per month?
Starter is $49, Essential is $79 and Plus is $99 a month at list price, with each additional clinician at $74 on the Plus plan. The AI Note Taker adds $35 per clinician per month and card processing is 3.15% plus $0.30. Those figures are from SimplePractice's pricing page on October 28, 2026.
What is the real cost for a group practice?
Multiply out before you commit. Three clinicians on Plus with Note Taker is $352 a month, which is $4,224 in a full year once the introductory discount ends, plus roughly 3.15% of your card volume. The 50% off three months promotion makes year one about $370 cheaper than year two.
Can I add a second clinician on the Starter or Essential plan?
No. Additional clinicians are only available on the Plus plan, so a two person practice starts at $99 plus $74, which is $173 a month before any AI add-on. That step from a one clinician plan to the group plan is the biggest single jump in the price list.
Does SimplePractice include an AI scribe in the subscription?
No. Note Taker is a paid add-on at $35 per clinician per month, free for the first 30 days, with a promotional rate of $17.50 shown on the pricing page. Care Aide bundles Note Taker with its other AI tools at $59 for the account owner and $49 per additional clinician.
Is SimplePractice worth it for a cash-pay practice?
It works, but you are funding insurance infrastructure you will not use. A self-pay practice needs charting, scheduling, payments, reminders and a clean superbill, and that stack costs less from vendors that never built a clearinghouse. If you might take insurance later, the flexibility can justify it.
What do most people switch to when they leave?
It depends entirely on insurance. Practices that still bill payers tend to move to TherapyNotes or Jane. Practices that have gone fully self-pay move to cheaper per seat tools, ours included, mainly because group pricing is the thing that pushed them to look.
The bottom line
SimplePractice publishes its prices, which is more than some of its competitors do, and the plans are reasonable for what they contain. The two numbers to hold in your head are $74 per additional clinician on Plus and 3.15% plus $0.30 on every card. Those two scale with your practice. The $49, $79 and $99 headlines do not.
Decide the insurance question first. Everything else is a spreadsheet.
DrinCloud is practice management software for clinics that charge the patient directly: AI charting, scheduling, payments, reminders and superbills, with one to three users on a $119 plan and no claims engine in the price. Compare it against SimplePractice line by line.
