IntakeQ and PracticeQ pricing: what $84.90 a month buys, and what it doesn't

IntakeQ is $54.90 and PracticeQ $84.90 a month, plus $20 or $30 per practitioner. Best intake forms on the market, and no AI scribe at all in 2026.

October 28, 2026 · 9 min read

IntakeQ sells two things under one roof. IntakeQ, the forms only product, is $54.90 a month plus $20 for each additional practitioner. PracticeQ, the full practice management product, is $84.90 a month plus $30 for each additional practitioner. There are also reduced rates for practices with lower client volumes, $29.90 for forms only and $59.90 for practice management, which the public page mentions as discounted plans without printing the figures, so you have to ask (forms.intakeq.com/pricing, read October 28, 2026).

What $84.90 buys is the best patient intake experience in this price bracket, by a clear margin: conditional logic, forms that look like a real clinic made them, signatures inside the form, and an intake flow patients finish on a phone without calling you for help. What it does not buy, in October 2026, is an AI scribe. There is no ambient recording, no draft note from a conversation, no per-minute tier. The page lists an AI form builder, which is a different thing: it helps you write the form, not the note. In a year when a scribe costs $15 to $40 a month almost everywhere else, that is the single biggest hole in the product and nobody selling it to you will lead with it.

What IntakeQ and PracticeQ cost

ProductBase per monthAdditional practitionerLow volume rateAI scribe
IntakeQ, forms only$54.90$20 each$29.90none
PracticeQ, practice management$84.90$30 each$59.90none

Two things to understand about that table before you compare it to anything.

First, the two products are not tiers of the same ladder in the way Starter and Plus are at other vendors. IntakeQ is a forms and intake tool you can run alongside software you already have. PracticeQ is scheduling, charting, invoicing, payments and a client portal, with the same forms engine inside it. Plenty of practices buy IntakeQ purely to replace paper intake while keeping their existing system for everything else, and that is a legitimate $54.90 purchase.

Second, the per practitioner charge is the number that decides your bill, and $30 is middle of the market. It is three times Noterro's $13.20 and well under SimplePractice's $74 on the Plus plan. At two or three clinicians it keeps PracticeQ competitive. At six it is $150 a month of seats on top of the base.

What $84.90 actually gets you

The honest version, feature by feature, is that PracticeQ covers the full administrative loop of a small practice: online booking, appointment reminders, treatment notes, invoices and payments, a client portal, packages, and the intake forms that are the reason most people arrive. Support is included, as it is with every vendor on this list, so "support included" tells you nothing when you compare.

The forms are the part worth paying for. If your practice has a long intake, a functional medicine history, a pelvic health questionnaire, a multi page nutrition assessment, then the quality of the form builder stops being a nice to have and becomes the thing that saves you fifteen minutes per new patient. Conditional questions that hide themselves, required fields that actually block submission, consent and signature in the same flow, and a patient facing result that does not look like a 2009 web form. That is a genuinely best in class piece of software and we will not pretend otherwise.

Signature is included in the form flow, which matters because the alternative in a lot of systems is a separate e-signature product with its own bill. For the record, remote e-signature where the patient signs on their own phone is included on every plan in our software too, and if a vendor charges you extra for that in 2026, push back.

The missing AI scribe, and what it costs you to work around

IntakeQ has no scribe. If you want ambient documentation you buy a standalone one and paste the note in. That is a real workflow, thousands of clinicians do it, and here is what it adds to the monthly bill, using list prices read on October 28, 2026: Freed $59 to $119, Twofold $69, Tebra $99, Charm $125, Sunoh $149. Heidi, Nabla and Suki do not publish a price. Cerbo's is $29 or $59, but that is bundled with their platform.

So PracticeQ plus a standalone scribe for one clinician realistically lands between $144 and $184 a month, and the note arrives by copy and paste rather than attached to the encounter you just charted. Compare that with scribes that live inside the chart: Jane at $15 per practitioner, Practice Better at $10 to $40, SimplePractice at $35, and ours at $20 per clinician.

Setup, one clinicianMonthly
PracticeQ alone$84.90
PracticeQ plus Freed at $59$143.90
PracticeQ plus Twofold at $69$153.90
PracticeQ plus Tebra at $99$183.90
A platform with the scribe built in, at $20 per clinician$69 to $139 depending on the platform

The dollars are only half of it. A scribe that is part of the chart writes into the note you are already in, and the draft carries the patient and the visit with it. A scribe in another tab does not. If documentation time is your actual problem, read how the time math works out in what an AI scribe does to your note writing time before you pick either route.

The bill at one, three and six practitioners

Everything below is list price from each vendor's public pricing page on October 28, 2026, with the AI scribe turned on wherever the vendor sells one, because that is the realistic 2026 setup. SimplePractice does not publish a per practitioner figure on Essential, so the three and six clinician rows use the Plus plan at $99 with additional practitioners at $74 and the Note Taker at $35 each.

PractitionersPracticeQSimplePracticeDrinCloudAI included in those totals
1$84.90$84 on Starter with Note Taker$69 on EssentialPracticeQ no, others yes
3$144.90$352 on Plus$179 on ClinicPracticeQ no, others yes
6$234.90$679 on Plus$407 on Medical CenterPracticeQ no, others yes

Read the six practitioner row and notice who loses. At six clinicians PracticeQ at $234.90 is cheaper than we are at $407, because our Clinic plan covers one to three users, users four and five are $45 each, and a sixth user moves you to the Medical Center plan at $287. If you have six clinicians, no insurance billing and you are content to bolt on a scribe or do without one, PracticeQ is the better buy on price and we are not going to argue the point.

Where the picture flips is one to three clinicians with AI included, and it flips hard against SimplePractice rather than against IntakeQ. $352 versus $179 at three clinicians is not a rounding difference, it is a car payment. The full SimplePractice price list, including the promotion that makes the first three months look cheaper than the product is, is in our SimplePractice pricing breakdown.

IntakeQ vs SimplePractice, decided in four lines

  1. Forms. IntakeQ wins, clearly, and it is not close enough to debate.
  2. AI documentation. SimplePractice wins by default, because IntakeQ has none.
  3. Insurance billing. SimplePractice wins. It has claims, ERA and a clearinghouse. IntakeQ handles superbills and some claim workflows, but if claims are your revenue, SimplePractice is the safer platform.
  4. Price at three clinicians. IntakeQ wins by a wide margin, $144.90 against $352.

If you bill insurance, stop here and buy SimplePractice or TherapyNotes. Everything we build assumes the patient pays you directly, and the honest comparison for that case is on our side by side comparison page.

Where each one is the right answer

Buy IntakeQ forms only at $54.90 if your intake is the broken part and the rest of your stack works. It is the cheapest fix for the worst part of most practices.

Buy PracticeQ at $84.90 if you want one system for scheduling, notes, invoices and the best forms available, you have two to six clinicians, and you either do not want a scribe or you are happy pasting one in.

Buy something with a built in scribe if documentation time is what is actually hurting. At one clinician that is $69 to $134 a month depending on the platform, and the note lands in the chart instead of your clipboard.

Buy a cash-pay platform if you have no claims at all. Insurance machinery you never use still costs money and still adds fields to every screen. That is the argument behind DrinCloud, where charting, superbills, scheduling, payments and a multilingual patient portal start at $49 a month and there is no clearinghouse anywhere in the product.

Three things we do not do, so you can rule us out fast: no insurance claims, no ONC certification, no e-prescribing, which matters in states with an e-prescribing mandate such as California, Florida, New York and Michigan. Our plan and price list says all of this on the page rather than in a sales call, and the AI scribe is a $20 per clinician checkbox, not a quote.

If you decide to leave IntakeQ

Export first, then cancel. IntakeQ's own migration documentation is a useful model of what to expect in either direction: for a move from SimplePractice into PracticeQ, their help article says only client demographics and charts come across, and tells multi clinician practices to run one export per clinician so that notes land under the right author (support.intakeq.com). Expect the same shape of limits on the way out. Forms you built are the asset least likely to travel, so download the completed PDFs for every active patient before your last billing date, and keep them.

A general process, with the order of import and the thirty day overlap that stops you losing anything, is in how to switch EHR without losing your data.

Frequently asked questions

How much does IntakeQ cost per month?

IntakeQ, the forms only product, is $54.90 a month plus $20 per additional practitioner, and PracticeQ, the full practice management product, is $84.90 a month plus $30 per additional practitioner, as listed on IntakeQ's pricing page on October 28, 2026. Reduced rates of $29.90 and $59.90 exist for practices with lower client volumes.

Does IntakeQ have an AI scribe?

No. As of October 2026 neither IntakeQ nor PracticeQ includes an ambient AI scribe that records a visit and drafts the note. The AI form builder on the pricing page helps you create forms, not notes. To get ambient documentation you have to buy a standalone scribe, which typically adds $59 to $149 a month.

Is PracticeQ cheaper than SimplePractice?

At every practitioner count we priced, yes. One clinician is $84.90 against $84 for SimplePractice Starter with Note Taker, three clinicians is $144.90 against $352 on SimplePractice Plus, and six is $234.90 against $679. The gap comes from the per practitioner charge, $30 at IntakeQ versus $74 on SimplePractice Plus.

What is the difference between IntakeQ and PracticeQ?

IntakeQ is the intake forms product: questionnaires, consents, signatures and the patient facing flow. PracticeQ is the complete practice management system, with scheduling, charting, invoicing, payments and a client portal, and it contains the same forms engine. They are priced separately at $54.90 and $84.90.

Can PracticeQ bill insurance?

PracticeQ supports superbills and claim workflows, so a practice with occasional out of network billing can work with it. It is not as deep as a platform built around claims, so if the majority of your revenue is adjudicated by payers, compare TherapyNotes and SimplePractice first.

What happens to my forms if I leave IntakeQ?

The structure of a form you built is the least portable thing you own and should be treated as unrecoverable. Completed submissions can be downloaded as PDFs, so export the current ones for every active patient before your subscription lapses, and plan to rebuild your intake from scratch in the new system.

Our software is built for practices the patient pays directly: AI charting, superbills with your own CPT and ICD-10 codes, remote e-signature and online booking from $49 a month, with no claims engine and no sales call. See what is in each plan.
